Rights

You are a vital member of your health care team. So it’s important that you understand your rights and responsibilities.

These rights and responsibilities ensure that every patient and their family have a safe and positive experience while receiving care from our care team. We will respect your right to:

Active Involvement: Be actively involved in your care to the extent you are comfortable, competent, and capable.

Dignity and Respect: Be treated with courtesy and respect.

Privacy: Receive appropriate privacy and confidentiality.

Designate a Substitute Decision-Maker: Designate a person to exercise rights on your behalf if you cannot make health care decisions for yourself.

Responsibilities

Mutual respect and cooperation are essential to the delivery of quality health care. To successfully meet your health care needs, we ask that you and/or your representative(s) please:

  • Treat others with respect and dignity.
  • Consider that other patients may also need help.
  • Use a calm voice and non-threatening body language.
  • Report unsafe conditions.
  • Understand your healthcare needs.
  • Let your healthcare team know when you don’t understand. Ask questions and express concerns.
  • Provide relevant information to your health care team.
  • Maintain the confidentiality of other patient’s health information.
